If you are approved for the Lifespan Respite Subsidy Program, you will receive a packet of information in the mail. This packet includes a welcome letter with your Client ID and billing documents for the year. You submit one billing document for each provider each month. You can click here for a fillable document that you can print out.
Be sure that you fill in every box. If you do not have your provider’s payee number, please call 308-432-8190 and Sherri Blome can locate this for you.
The provider rate can be by the day, or by the hour. Please see the sample billing document here.
You can submit your billing documents to WCHR/Respite, 300 Shelton St., Chadron NE 69337 or fax to 308-432-8974. This allows the respite coordinator to review the document for errors before submission to DHHS. You will be notified when the document has been submitted and when it has been paid.
